During your first visit you will have a one-on-one consult with your clinician about your case. This is to ensure that you are a good candidate for treatment at the clinic, and that you are familiar and comfortable with our philosophy and program. At the conclusion of your free consultation, you will be presented with a comprehensive treatment plan and have the opportunity to proceed with your first acupuncture treatment that day or schedule an appointment on another day.Enter Dates and Times that work for you, and any other information you think we should know!

If you have an email address that was put into our system, please check there for an email with the subject line "Appointment Reminder" and you can find your new patient forms there. Please get them filled out and signed before your first appointment with us. If you do not have access to an email address, please print these out, fill them in, and bring them to your primary appointment.
