Please arrive 30 minutes early for your first appointment to fill out the necessary paperwork which includes a thorough health history. 24 hour Cancellation Policy : If you have to cancel or reschedule your appointment, please remember to give us a minimum of 24 hour notice, cancellation must be during business hours, to avoid the cancellation fee charge. We are not participating providers with any insurance companies at this time.
